Home Health Visits with Growth Potential
Job Summary:
We are seeking a motivated and skilled Physical Therapist (PT) to join our team. This is a per diem or contract position focusing on home health visits. The ideal candidate will deliver high-quality care to patients in the comfort of their homes while adhering to Medicare and Medicaid standards.
Key Responsibilities:
- Provide physical therapy assessments, interventions, and care planning for home health patients.
- Perform admissions, evaluations, routine visits, and discharges per Medicare and Medicaid guidelines.
- Educate patients and caregivers on therapy plans, techniques, and home exercise programs.
- Document patient progress accurately and in a timely manner.
- Collaborate with interdisciplinary teams to ensure quality care.

Remote Therapeutic Monitoring (RTM):
- Utilize RTM technology to monitor and manage patient progress remotely.
- Ensure compliance with RTM CPT codes and Medicare billing requirements.
Outpatient Physical Therapy Services:
- Provide therapy services in a clinic setting or through telehealth.
- Deliver personalized treatment plans for musculoskeletal and other conditions.
Requirements for RTM and Outpatient Physical Therapy:
- Willingness to obtain a Medicare Provider ID.
- Services will be billed under a group using the group’s Provider ID.
