Job Summary:
We are seeking a compassionate and experienced Medical Social Worker (MSW) to provide home health services, supporting patients and families in navigating healthcare and social needs.
Key Responsibilities:
- Assess patients’ social, emotional, and financial needs to create care plans.
- Provide counseling and connect patients with community resources and support services.
- Collaborate with interdisciplinary teams to ensure continuity of care.
- Advocate for patients' needs with payers, providers, and other entities.
- Maintain compliance with Medicare and Medicaid documentation requirements.
