A Physical Therapist (PT) is needed for a full-time/Part time in Home Care position in Cleveland OH.

Develop treatment plans by identifying a problem list and methods to achieve identified goals.
Coordinate treatment plan with the patient, family, physician, and other team members.
Adapt treatment to meet the individual needs of the patient.
Reviews the quality and appropriateness of the total services delivered.
Supervisor PTA's and HHA's experience.
Manual Therapy: 1 year (Required).

License/Certification:

Physical Therapy License (Required).
