Home Health Visits with Growth Potential

Job Summary:
We are seeking a motivated and skilled Physical Therapist (PT) to join our team. This is a per diem or contract position focusing on home health visits. The ideal candidate will deliver high-quality care to patients in the comfort of their homes while adhering to Medicare and Medicaid standards.

Key Responsibilities:

Provide physical therapy assessments, interventions, and care planning for home health patients.
Perform admissions, evaluations, routine visits, and discharges per Medicare and Medicaid guidelines.
Educate patients and caregivers on therapy plans, techniques, and home exercise programs.
Document patient progress accurately and in a timely manner.
Collaborate with interdisciplinary teams to ensure quality care.

Remote Therapeutic Monitoring (RTM):

Utilize RTM technology to monitor and manage patient progress remotely.
Ensure compliance with RTM CPT codes and Medicare billing requirements.

Outpatient Physical Therapy Services:

Provide therapy services in a clinic setting or through telehealth.
Deliver personalized treatment plans for musculoskeletal and other conditions.

Requirements for RTM and Outpatient Physical Therapy:

Willingness to obtain a Medicare Provider ID.
Services will be billed under a group using the group’s Provider ID.
